This Phoenix, Arizona woman had a long course that began with a spinal anesthetic which developed complications. She had spent 14 years in a wheelchair, and gained a lot of weight, which compounded the situation. Her highest weight was almost 400 pounds at 5' 7"! However, she was ultimately able to lose more than half of her body weight, to about 160 pounds, which she was able to maintain over two years. She had previously had a circumferential abdominoplasty by another surgeon, which produced some tightening of the buttocks, but still had very large thighs, and she said she felt "like a monster more than a fat girl." Because there was still a lot of fat in the thighs, the first procedure Dr. Nachbar did was liposuction, to remove over 3,600 cc (about eight pounds of fat!). The second procedure by Dr. Nachbar was an extended inner thigh lift. However, as you can see in the last pair of pictures below, that still left extra tissue on the outside of the thighs at the level of the bottom of the buttocks. Therefore, Dr. Nachbar performed a lower buttock tightening procedure. This procedure is not as common as other thigh lifts, but is perfect for this situation.
